The Community Record aids Long Term and Post-Acute Care Providers

Care of the most complex patients often requires stays in Long Term and Post-Acute Care (LTPAC) facilities. Most patients referred to  LTPAC facilities in our region received care from multiple providers. After a joint replacement in Anacortes they may require several nights of LTPAC stay post discharge. Their care record may include a history o cardiac care from Mount Vernon-based providers.

The MIN-NS Health Information Exchange provides access to individual reports of care from throughout the area with labs, operative reports, echocardiogram reports, imaging reports available in one place with one sign on. Even more useful is the ability to create a single, downloadable Clinical Summary document which includes all available medications, problems, allergies, lab results, reports and encounter records for the patient for the last 6 months.

It is this access to a single source of patient information from across the Valley that has made local LTPAC facilities to be among the heaviest users of the HIE.
